Organisational Overview
Neami is a national not-for-profit mental health organisation that has been providing psychosocial rehabilitation and support to people with a mental illness for over 20 years. A range of federally and state funded programs are offered at 31 service sites in Australia.

Neami Whittlesea is based in South Morang, consisting of a team of Community Rehabilitation and Support Workers, Peer Support Worker, Senior Practice Leader and a Service Manager. It provides a variety of services and programs including Home Based Outreach Service, which provides individual rehabilitation and support to consumers in their own homes, or in accommodation allocated to Neami by the Office of Housing, and Group Rehabilitation Programs, the content of which is based on collective need, identified in the assessment process. Also among the services offered are Day to Day Living (D2DL) in the Community and Pathways. Pathways is designed for consumers of the Northern Psychiatric Unit at risk of homelessness, or who face homelessness upon discharge, and aims to assist consumers into stable and appropriate accommodation. D2DL is designed for consumers with severe and persistent mental illness. The aim of the program is to improve health outcomes for individuals with severe and enduring mental illness who experience social isolation by providing structured and socially based activity programs to assist people to participate in social, recreational and educational activities in the community.
Position Overview
The Pathways Housing Worker will be based part-time at the Northern Hospital and part-time at Neami Whittlesea as negotiated. They will have a small CRSW case-load as well as being primarily the Pathways housing and homelessness worker. They will work closely with the social work team at the Northern Psychiatric Inpatient Unit. They will also be expected to meet regularly with the project Steering Group, to write monthly reports to the Victorian State manager and to meet the reporting criteria laid down by the Department of Human Services.
Period of Employment
12-month maternity leave position.
